2022- 23 DIAA Sports Physicals & Fall Sports Info
Fall Sports Begin on:
Football- August 8th
XC, Field Hockey, Volleyball, Boys Soccer- August 15th
Unified Flag Football- TBD
Middletown High School football cheerleading is looking for more incoming freshman to join our FRESHMAN TEAM! No cheer experience required! Email Coach Clay at alexis.clay@appo.k12.de.us if you're interested.
Every student athlete NEEDS a new 22-23 DIAA physical on file for this school year in order to try out for ANY High School Sport.
If your child would like to participate in high school sports next school year, he/she will need a 22-23 DIAA Sports Physical completed by their physician or qualified healthcare provided after April 1, 2022 to be eligible for tryouts and conditioning next school year. If signatures are missing from any page or forms are incomplete then the student is not eligible to participate.
Fall 2022 Sports: DIAA Physicals for Fall 2022 sports will be collected at the high school your child is attending at the dates and times listed below. These dates are the only times physicals will be collected during the summer months. No sports physicals can be emailed, faxed, or dropped off at the main office during the summer. Fall sports tryouts begin in early August and all physicals must be submitted at least two weeks prior to the first day of try-outs.
- June 13th from 8:30 am- 11:30 am
- June 14th from 1:00 pm-3:00 pm
- July 13th from 10:00 am-2:00 pm
- August 3rd from 9:00 am-11:00 am & 2:00-5:00 pm
- August 10th from 2:00 pm- 6:00 pm
DO NOT WAIT to get it done: The Middletown HS Wellness Center can do these physicals NOW for FREE for all our athletes. You can schedule an appointment with them by calling 302-696-3140 or your student can send a message on Schoology to "Susan Gilbert Wellness Center" to request an appointment. They will be open until June 7th to get these done.

Update: New 22-23 DIAA Sports Physical Forms
Not sure if your child has a completed DIAA Sports physical on file for this year?? Click HERE If their name is not listed we do not have one on file and they are not eligible to try out for a sport.
We encourage you to check with your medical home (Primary Care Provider) if you need a clearance before setting up something elsewhere. We also encourage you that when your child has their annual wellness visit to ask their doctor about DIAA pre-participation clearance.
DIAA Sports Physical:
All athletes must have a completed DIAA physical form prior to the first practice.PHYSICALS FOR THE 2022-23 SCHOOL YEAR MUST BE DATED AFTER APRIL 1, 2022.
1. Make sure that you make a copy of all the forms and keep a copy for your records before turning it in.
2. Make sure all signatures are in place, if not the forms are invalid. You will not be allowed to participate in any activities until all necessary forms are complete.
3. A s long as the athlete has had a routine physical exam (Well visit) within the last 12 months with their Primary Care Provider (PCP), their PCP can fill out the DIAA forms stating their clearance is anytime after April 1st. The exam date can be anytime within the last year.
WHERE CAN I GET A PHYSCIAL??
1. The Middletown School Based Health Center is open and scheduling appointments for students who will be participating in sports for 2022-2023 school year. The physicals are convenient and free to Middletown students.
